This form must be completed by the fireworks contractor prior to conducting any fireworks display. Under Section 169 of the Explosives Regulation 2017, Resources Safety & Health Queensland (RSHQ) must be notified in the approved form, of any fireworks display at least 7 calendar days prior to conducting the display.
