Form: Privacy Complaint_
You can use this form to submit a privacy complaint to the Office of the Australian Information Commissioner (OAIC).
The OAIC can investigate privacy complaints from individuals about Australian and Norfolk Island Government agencies, and private sector organisations covered by the Privacy Act 1988 (Cth). You can also make a complaint to the OAIC about the handling of your personal information by ACT public sector agencies under the Information Privacy Act 2014 (ACT).
The OAIC uses the Australian Government’s SmartForm service which enables you to lodge our forms online using a personal computer or mobile device. When you save or submit a form using this service, it is encrypted and stored on a secure server located in Australia and controlled by the Department of Industry. Once you have submitted your completed form, we will download and decrypt it. Your form will then be deleted from the Department of Industry's server within 48 hours.
The Department of Industry will not be able to decrypt and access your form.
You can save this form at any point and return to complete it within 28 days. If you have not submitted your form within 28 days it will be automatically deleted from the Department of Industry's server.
