Who can complete this form?

This form should be completed by a director or the authorised officer of the applicant company.

Unless stated otherwise, all section numbers are references to the Duties Act 2000.
The corporate reconstruction exemption applies to eligible transactions occurring on or after 1 January 2004 between members of a corporate group.

An application will not be processed unless all Parts of this form are completed in full and the documents listed in 'Document checklist' section are provided. If there is insufficient space, please attach a schedule.
The SRO may contact the applicant or the applicant’s representative for further information.

How is this form lodged?

This form is completed online. When you have completed all relevant and mandatory information, please select submit and download a copy of the completed form. Print and sign a copy of the completed form and send it, together with any supporting documentation, to:
