What is the purpose of this form?

The purpose of this form is to nominate the Designated Group Employer. Any previous nominations or designations will be revoked from the effective date.

Who can complete this form?

This form should be completed by the Designated Group Employer or their authorised representative. Only one member of a group (Designated Group Employer) can claim the Victorian payroll tax statutory threshold entitlement.
